Karen is our in-house claims specialist; she will guide you through the claims process. When a claim occurs during business hours, call our office at 402-475-3213 and ask for her. Please have claim information and your policy number on-hand.
If you have a claim during our non-business hours, you will find a list of our most commonly used companies and their respective claim hotlines at the bottom of this page. Clicking on a company logo will take you to the company's claim page. Alternatively, you can use one of our claim forms at anytime.
The teal items are downloadable forms to be used for complete claim submissions. The Quick Claim Form button will allow you to simply notify Karen that you have a claim, and she will contact you for further information. Please use the form the corresponds best to your loss and situation:
DOWNLOADABLE FORM DIRECTIONS: Download and complete the appropriate form(s) above for your claim. For your safety, do not fill out the forms while they are in the browser. When you have completed a form, click ‘Submit Form’ at the top of the document. If you have a desktop email application, it will create an email for you. If you use an internet-browser-based email, clicking 'Submit Form' will save the document so you can attach it to an email. Please send claim forms to Karen@CoppleInsurance.com.
